DO YOU NEED HELP WITH YOUR MEDICAID OR CHILD HEALTH PLUS APPLICATION?

Are you confused by your Health Insurance options?
Do you need help with your Medicaid or Child Health Plus Application?

Funded by the New York State Department of Health, the Facilitated Enroller (FE) program is free and helps to make sure children and adults have access to comprehensive, affordable health insurance. You can learn more about Child Health Plus, Family Health plus and Medicaid through Facilitated Enrollers at locations in your community.
The facilitated enroller will advise you on which program you are eligible for, what documents you should present and how to fill out the application.

There are four organizations in Wayne County providing this service: Wayne County Department of Social Services; the S2AY Network; Thompson Health Network; and Fidelis Care.
The first step is always to call one of the numbers in the list below. You will find out which documents to bring; and you will be able to make an appointment. During the appointment, the facilitated enroller will help you fill out the application, check your documents and make copies, as required. The enroller will forward your application to the appropriate agency and encourage you to call again, should you receive any letters from the insuring agency or the Department of Social Services. You do not need to go to DSS to apply for NYS sponsored insurance.

Please note: starting April 1, 2010, you can mail completed applications for Medicaid directly to:

In-person interviews are not required anymore.

DO YOU NEED HELP UNDERSTANDING MEDICARE?

65 AND OLDER? NEED HELP UNDERSTANDING MEDICARE? The Wayne County Department of Aging and Youth is continuing its very popular "Medicare 101" classes through 2010. Medicare is available to most people at age 65, but may be available earlier than that to individuals who have disabilities. If you're new to Medicare or will be joining in the next few months, sign up for a class and learn more about the Medicare enrollment timeline, an overview of coverage options, Medicare Advantage plans, supplemental insurance policies, Medicare savings programs, and Medicare Part D - the newest Medicare program that helps to cover prescription drug costs. Classes run approximately 1½ hours and are held at the Department of Aging and Youth, located in the Health Services Building at 1519 Nye Road, Lyons. Light refreshments will be provided. Classes are scheduled for Wednesday, May 5 at 2:00 p.m.; and Tuesday, June 8 at 10:00 a.m. Classes are free, but registration is required.
