Creating Healthy Places to Live, Work and Play is an initiative funded
by the New York State Department of Health to work with local organizations
and families in Wayne County to prevent obesity. Link to
Creating Healthy Places website for calendar of events, activity tips,
recipes, resources and partners. Workshops on prevention of obesity,
diabetes and picky eaters, also cooking for one, portion distortion and
others. Also programs to increase physical activity and healthy lifestyle
choices. Eat Smart NY is an educational program to provide parents,
individuals, seniors and youth hands-on, interactive experiences to demonstrate
ways to improve healthy eating and physical activity.

A federally qualified community and migrant health center providing
comprehensive primary care, including medical, dental, and behavioral
health services. Full dental services including access to Eastman
Pedodonist via tele-dentistry. Full medical services including: acute care,
chronic disease management, women.s health, men.s health, family planning,
preventive services (screenings, immunizations, well child visits,
school and work physicals, nutrition education, tele-retinopathy screening),
and referrals. Case management/health advocates for migrant and
seasonal farmworkers, minority, and underserved populations. Behavioral
health services. Medicaid, Family Health Plus, Child Health Plus,
Medicare and most other medical and dental insurances are accepted.

WIC provides healthy foods, nutrition education and breastfeeding support
to eligible women, infants, and children less than five years old.
WIC foods include fruits, vegetables, bread, rice, tortillas, milk, cheese,
cereal, juice, eggs, peanut butter and more. Infant formula is available
for those who supplement or choose not to breast feed. Eligible persons
must live in NYS and meet income eligibility guidelines (families enrolled
in Medicaid, Food Stamps, TANF, Head Start, or Free/Reduced
Price School Lunch are automatically income eligible for WIC). Mon.-Fri. 8am-4:30pm, evenings by appointment.
