A federally qualified community and migrant health center providing
comprehensive primary care, including medical, dental, and behavioral
health services. Full dental services including access to Eastman
Pedodonist via tele-dentistry. Full medical services including: acute care,
chronic disease management, women.s health, men.s health, family planning,
preventive services (screenings, immunizations, well child visits,
school and work physicals, nutrition education, tele-retinopathy screening),
and referrals. Case management/health advocates for migrant and
seasonal farmworkers, minority, and underserved populations. Behavioral
health services. Medicaid, Family Health Plus, Child Health Plus,
Medicare accepted. Sliding fee scale available. Interpreters available.

The Migrant and Seasonal Farm Worker Health Program provides services
and coordinates services to Wayne County.s Migrants and Seasonal
farm workers and their families. Mission is to provide a bridge
between the providers of health, social and community services for this
under served and hard to reach population in our Wayne County community.
In Camp or Home Nursing Care, Health Screenings, Health
Education, Maternal-Child Health Guidance visits, Communicable Disease
follow up, Case Management Services including Interpretation and
Transportation, Clinics for Health Screening & Immunization,
Mammogram.s, PAP smears, STD clinics, TB testing, Well Child Clinic.

Assistance programs for agricultural workers, minority health assistance.
Wayne RX Card, RX card offering up to 50% discount on all brand
pharmaceuticals at all Wayne County Pharmacies. Cannot be combined
with existing RX insurance. Provide training for Chronic Disease Self
Management to empower residents to better deal with their own illnesses.
Provides a community based geriatric assessment program, case management
and individualized assistance.
