A federally qualified community and migrant health center providing comprehensive primary care, including medical, dental, and behavioral health services. Full dental services, including access to Eastman Pedodonist via tele-dentistry. Full medical services including: acute care, chronic disease management, women.s health, men.s health, family planning, preventive services (screenings, immunizations, well child visits, school and work physicals, nutrition education, tele-retinopathy screening), and referrals. Case management/health advocates for migrant and seasonal farmworkers, minority, and underserved populations. Behavioral health services include mental health counseling and telepsychiatry consultation. Medicaid, Family Health Plus, Child Health Plus, Medicare accepted. Sliding fee scale available.
