Help with application: If you would like someone to assist you in
applying for either Children.s Medicaid or Child Health Plus, there are
many enrollers in our community to help, FOR FREE. These healthcare
advocates called Facilitated Enrollers will help you fill out the application,
collect the necessary documents, and select a health plan. Assistance is
available during evenings and weekends. FEs in Wayne County are:

Applying without an advocate: This one application can be used to
apply for Children's Medicaid, Child Health Plus, Family Health Plus,
Medicaid, Prenatal Care Assistance Program (PCAP), and Women, Infants
and Children (WIC) nutritional programs. Based upon the information
you provide, you will be told which program you and/or your
child(ren) are eligible for. You can get a hardcopy of this application by
calling 800.698.4543 or by emailing chplus@health.state.ny.us. Please
note: starting April 1, 2010, you can mail completed applications for
Medicaid directly to: Wayne County Department of Social Services,
77 Water St., Lyons, NY 14489. In-person interviews are not required
anymore. All appointments are free.

Children's health insurance explained: New York State has a health
insurance plan for kids, called Child Health Plus. Depending on your
family's income, your child may be eligible to join either Children's
Medicaid or Child Health Plus. Both Children's Medicaid and Child
Health Plus are available through dozens of providers throughout the
state. To be eligible for either Children's Medicaid or Child Health Plus,
children must be under the age of 19 and be residents of New York State.
Whether a child qualifies for Children's Medicaid or Child Health Plus
depends on gross family income. Children who are not eligible for Medicaid
can enroll in Child Health Plus if they don.t already have health
insurance and are not eligible for coverage under the public employees'
state health benefits plan. Some children who were covered by employer-
based health insurance within the past six months may be subject
to a waiting period before they can be enrolled in Child Health Plus.
Determining whether your child is subject to a waiting period will depend
on your household income and the reason your child(ren) lost
employer-based coverage.
What is covered: Coverage includes health needs like: physical exams,
immunizations, diagnosis and treatment of illness and injury, X-ray and
lab tests, outpatient surgery, emergency care, prescription and nonprescription
drugs if ordered, inpatient hospital medical or surgical care,
short-term therapeutic ou outpatient treatment for alcoholism and substance
abuse, and mental health, dental care, vision care, speech and
hearing and durable medical equipment, emergency ambulance transportation
to a hospital and hospice.
